The Care Manager position maintains full caseloads of families and meets with each on a monthly basis. The goal is to rapidly engage and sustain engagement of the families throughout course of treatment using assessment tools appropriate to the position. CHHUNY is New York State’s largest children's Health Home, enrolling over 30% of all children in the Health Home program.

Census data, this includes about 207,000 children in those counties, and at least 135,000 children enrolled in 96 school districts across those counties.
