A Health Home is a community-based care management service model whereby all of an individual's caregivers communicate with one another so that all social, physical, and behavioral needs are addressed in a comprehensive manner. Northern Rivers Childrens Health Homes program is seeking a full-time Care Manager. This position is responsible for engaging children, youth, and families via outreach and enrollment activities for successful enrollment in a childrens health home and the subsequent provision of health home services.

The Care Manager position maintains full caseloads of families and meets with each on a monthly basis. The goal is to rapidly engage and sustain engagement of the families throughout course of treatment using assessment tools appropriate to the position. CHHUNY is New York State’s largest children's Health Home, enrolling over 30% of all children in the Health Home program.
Census data, this includes about 207,000 children in those counties, and at least 135,000 children enrolled in 96 school districts across those counties. The Appalachian Children Coalition envisions a future where every child has access to the care that they require at home, in school, and in their communities. Must be able to work well with individuals of various backgrounds, age, ethnicities, life positions, and socio-economic statuses. Successfully complete CANS-NY training and pass the required testing with a minimum score of 70%. Timely completion and submission of all required documentation under the Waiver Program. Timely identify and effectively communicate all behavioral, emotional or medical needs.
